**Hours**: 25 hours

**Salary**: £15,000 - £16,250 pro-rata based on hours (£24,000-£26,000 FTE)

We are seeking a compassionate and dedicated Hospital Family Support Worker to join our team. In this role, you will have the rewarding opportunity to deliver vital support services to families. With a focus on providing non-judgmental and compassionate care, you will work closely with families of all ages, ensuring their needs are met and offering recommendations for additional support. **Role overview**
- Deliver Momentum support service in assigned hospitals (Royal Marsden - Sutton, Kingston Hospital, St George’s - Tooting, St Peter’s - Chertsey and Epsom Hospital).
- Provide compassionate and non-judgmental support to families of all ages reporting needs and recommendations for additional support.
- Regular and timely collaboration within the Family Support Team (Hospital & Community) managing and reporting on caseload.
- Establish and maintain professional relationships with clinical colleagues and other parties in hospitals.
- Regular attendance, representing Momentum and supported families at Multi-Disciplinary Team meetings in each of our hospital partnerships - reporting back as required.
- Record accurate notes on database.
- Deliver presentations in hospital to all professionals to raise awareness of Momentum.
- Maintain contact with families providing ongoing support, supported remotely, via various channels following Momentum guidelines.
- Input and maintain accurate records in our database.
- Work independently to plan regular hospital visits, in line with our operational plan and report back to Senior Family Support Worker.
- Refer families into Momentum’s support service, in line with the criteria required for referral
- Attend relevant training, meetings, and family support events.
- Support marketing and fundraising efforts by providing updates, photos, and case studies.
- Act as a professional representative of the charity.
- Report safeguarding concerns promptly.
- Other reasonable requests in line with requirements of the role.

**Person specification**

**Essential**
- Experience working directly with children and families and being responsive to their needs.
- Strong communication skills (both written and verbal).
- Excellent presentation skills.
- Ability to work independently, adapt to evolving service needs and manage resources effectively.
- Proficiency in using IT systems including Microsoft Office and databases.
- Collaborative and empathetic approach, working in a people-oriented and emotional environment.
- Trained in safeguarding.
- Reliable internet connection at home for effective remote working.

**Desirable**
- Experience of working in a charity (health or care)
- Demonstrated ability to work effectively within a team, including collaboration with relevant agencies such as NHS services and third sector organisations.
- Experience in bereavement or social work would be advantageous.
- Possession of a valid UK driving license with access to a car.
