Wolf Healthcare Worcestershire are searching for passionate Healthcare Workers to provide support within a supported living setting, or out in the community, for 3 adults with learning disabilities who have varying levels of needs in the Worcestershire area.

This could range from motivating someone to eat healthily or visit the gym, or providing support to a person with emotional problems.

**SUPPORT WORKER ROLE INCLUDES (but not limited to)**:
You will be passionate about ensuring the service delivered is person-centred in supporting clients to develop confidence, skills, and independence within their homes and local community. Typical supportive duties will include (but not restricted to):

- Maintaining homes
- Budgeting & shopping
- Planning & preparing meals
- Accessing activities in the community (study, volunteering, social & leisure activities)
- Maintaining relationships with family & friends
- Accompanying clients to appointments and/or on excursions
- Laundry & housework
- Support with medication

**WHAT WOLF HEALTHCARE WORCESTERSHIRE ARE LOOKING FOR IN THE IDEAL PERSON**:

- Knowledge & (or) experience in a Health & Social Setting - Residential, Supported Living, Day Services, etc
- Flexibility - able to work on a rolling rota
- Ability to work autonomously or as part of a team
- Excellent communication with people at all levels
- Able to complete good quality support records - detailing the activities of daily living
