**This is a fixed-term contract for 15 months. Our services are contracted, and we currently have funding for this contract for 15 months.**
**This is a really exciting time to join the organisation as additional funding has meant the charity will be recruiting talented people to a number of new roles throughout the New Year.**
We have the following roles available:
**Stroke Association Support Coordinator - 1 x 21 hours.**
**Stroke Association Support Coordinator - 1 x 30 hours.**
**Stroke Association Support Coordinator - 1 x 35 hours.**
This is an exciting opportunity to be involved in a brand new project in South Warwickshire. The project is funded by SQuIRe (Stroke Quality Improvement for Rehabilitation). Our Stroke Recovery Services support stroke survivors and their families from hospital to home using a person-centred approach to address the practical and emotional needs of stroke.
We’re looking for innovative, passionate and professional individuals with excellent communication, interpersonal and organisational skills to join us in delivering this project.
Key responsibilities will include:

- Working with the local Health and Social Services to receive referrals to support stroke survivors and their careers.
- Providing personalised information, advice and support.
- Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan.
- Providing regular reviews to support people in establishing and achieving their own personal goals.

The post holder will have experience/background in:

- Have a background in a caring and/or charity profession supporting people with disabilities.
- Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions.
- Have experience of working with people who may have additional communication support needs.
- Have the ability to use the basic Microsoft system.

To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
