An exciting opportunity has come available as a Care Navigator at Knights Care Ltd. Knights Care Ltd is a privately owned company operating since 2006, specialising in Dementia and Nursing care. We are looking for a confident individual who is committed, driven and efficient to work in as part of the team at Drovers Call Care Home.

**Job Role**:
As Care Navigator, you will have a broad range of responsibilities. These primarily include supporting the care homes with compliance and quality control of care plans. This will include the following key points:

- Quality control audits
- Audit care plans for the group
- Quality control - risk assessments

The Care Navigator will work closely with the Nursing and Care Teams to assess the needs of patients and determining what types of services are needed to ensure that individuals receiving care are safe, and suitable for them. The Care Navigator will report to the Deputy Managers, Clinical Lead and the Home Manager.

**Responsibilities**:

- Communicating face to face and over the phone with patients, family members and external professionals
- Writing and reviewing care plans for residents
- Dealing with prescription requests
- Taking on tasks from clinical staff
- Engagement with residents in a person-centred approach

**Experience**:

- A minimum of 2 years’ experience as senior carer or alternative.

**Job Type**: Permanent

**Salary**: £11.50 per hour
