Principal accountabilities The post-holder will coordinate all administrative aspects of the FH Service and be responsible for prioritising work, reviewing work practices, setting up, managing and ensuring effective running of FH clinics. The post-holder will communicate with multidisciplinary teams across the area to request and provide information. Managing FH Clinics The post-holder will be responsible for (when required): Coordination and management of FH Clinics ensuring that dates and venues for the weekly clinics are booked and FH Nurses, Patients and GP practices are aware of the details for the clinics Ensuring all referrals are tracked and highlighted for discussion when results are available Ensuring all clinics are set up on GP Practices EMIS database and rooms are booked Ensuring FH Nurses are aware of which patients have responded to appointment requests and what clinics they should be attending on a weekly basis Ensuring that case notes, diagnostic test results, x-rays, are available for each patient Ensuring an attendance register is kept, proformas outlining patient treatment options/decisions are completed Keeping orderly filing systems for FH documentation for review by Peer Review Teams Create and maintain the Medicine exchange database to report on results on a monthly basis Tracking and Case Management The post-holder will: Work closely with consultants, specialist nurses, clerical and other staff to deliver a coordinated, consistent tracking and information service Monitor patients progress on the pathway and check weekly to report information back to team Ensure patients are booked into next appointments in line with national standards & Trust policies, communicating with key personnel Liaise with other departments to ensure that patients results are sought in a timely fashion Coordinating typing of clinical and non-clinical correspondence within the service line and within set timescales Escalation of Actions The post-holder will: Monitor the agreed timescales in the Trusts Escalation Policy. Escalate actions at the various levels as outlined in the policy Ensure that all relevant information is provided to the staff responsible for initiating escalation and alerts.

**Reporting on DNAs The post-holder will**: Identify DNAs via the EMIS system and report these to key staff within the Trust. Ensure all DNAs are identified and an initial report sent weekly to the Operational Manager. Communication The post-holder will: Gather information from various sources to ensure that the benefits from FH clinics are identified Communicate with the FH Nurses to ascertain information and to update records, databases and clinics Organise, coordinate and present clinic information to the FH team meeting Communicate with Primary Care GP Practices to organise dates and times for FH Nurse presentations Communicate information of FH pathway to service managers and other staff at weekly tracking meetings Communicating with patients and patient relatives over the telephone, responding to their queries in a caring, sensitive and reassuring manner. Use initiative when dealing with and resolving patient enquiries, escalating as appropriate.

Being familiar with and following health and safety policy and procedures and to be aware of individual responsibilities under legislation, drawing any areas of potential risk to the attention of managers Ensuring completion of timely PDR/Appraisal process with designated line manager Being able to demonstrate Trust values and behaviours (outlined in staff handbook) at all times Undertaking any other duties which are deemed appropriate to the band The above indicates the main duties of the post which may be reviewed in the light of experience and development within the service. Any review will be undertaken in conjunction with the postholder. Education and training To contribute to and agree a continuing Personal Development Plan. To attend internal or external courses as agreed within the plan or ad hoc training and development as requested by management.

To attend on-going refresher training within their job role as legislation changes and upgrades to the PAS, EDMS or other local systems are introduced. Ensure mandatory training including Information Governance training is maintained within compliance date arranging time with line manager to complete when required. Participate in departmental meetings and awareness sessions as and when required. Employees are expected to develop their IT skills necessary to support the tasks included in their post.

They will, therefore be required to undertake any necessary training to support this. Working Relationships: Executive Directors, Divisional Head of Operations, Service Line Managers, Consultants and other clinicians, Information Staff, Finance Department, Human Resources, peers within other NHS Trusts
