**Your role as a Care Coordinator will include**:

- Care Coordinator to liaise with the Registered Manager to ensure that there is enough cover to deliver all visits and that new clients are planned into the rota in a timely fashion.
- Communicate, agree and record any schedule changes to clients and care professionals
- Monitor and Record carers sickness
- Ensuring staff rotas meet service users’ needs and aspirations
- Work with Recruitment Team to ensure sufficient current and future staffing levels
- To provide care services to service users in an emergency, arrange emergency cover when required and cover all/ any calls reasonable where required
- To ensure that the telephones are answered promptly, and people are spoken to in a polite and respectful manner
- To work alongside the Registered Manager ensuring that all quality standards, complaints, and comments are dealt with promptly
- Participate in the paid on-call rota and complete hand over notes.
- Put in place measure to comply with CQC regulations and work to achieve the best care standards possible
- Receive and handle complaints about services from service users or others;
- Carry out on-call coordinating duties outside normal office hours as agreed.
- Carry out risk assessments
- Assist the Registered Manager with writing and reviewing care plans
- Supervise care worker and report to Registered Manager
- Must have the ability to maintain clear and accurate written records and follow statutory reporting procedures
- Care Coordinator must be able to communicate clearly and build positive working relationships with clients, their families, fellow staff and other health and social care professionals.

**Requirements**:
We are looking for someone who is committed, honest, friendly, reliable, and respectful. In addition to these outstanding qualities, we are looking for a Domiciliary Care Coordinator to join us who is:

- 100% committed to helping improve the quality of life of vulnerable people
- Flexibility to cover on call to meet the needs of the business (essential)
- Knowledge of CQC standards and CQC Compliance (essential)
- Experience in Care Coordinating or 2 year Care Coordinator experience in the domiciliary care sector (essential)
- Full Driving Licence and access to a vehicle (essential)
- NVQ Level 3 or 4 in Health & Social Care (essential)
- Strong IT and logístical skills and Good knowledge and understanding of **People Planner /Mobizio** care rostering system an advantage
- Exceptional verbal and communication skill is essential
- To have good Organisational skills and Good geographical knowledge
