Healthcare Case Manager

My client has been established for 20 years and operate within the Occupational health sector, offering expertise in the Health, Wellbeing and Safety within the workplace. They are looking for an individual who is passionate about mental health and has experienceof supporting others.

**Key Duties**:

- Establish the current reason for ill health/injury and identify underlying causes or contributory risk factors via telephone assessment
- Use the stepped care model and the support options for each step
- Provide guided self-help via telephone / video conferencing
- Identify and escalate any safeguarding issues to line manager
- Support case managers with referrals for treatment
- Use an investigatory approach to cases and ensure obstacles are identified

**Essential Requirements**:

- English and Maths GCSE grade C
- Degree level education or Equivalent professional certification or extensive relevant experience
- Confident telephone skills using effective listening and empathy
- Knowledge of clinical and non-clinical pathways
- Previous case management experience is desired
- A passion for health & wellbeing
