An exciting opportunity has arisen for a social care practitioner to join the Discharge to Assess Team Inner City and East, and North and West.

The Discharge to Assess team works with people who have recently been in hospital or are receiving support such as reablement, inpatient rehabilitation or in a care home. The team consists of Social Workers, Social care practitioners and OTs (Occupational Therapy) who work closely together to make sure if ongoing support is needed, it is at the correct level and sustainable.

We work closely with other teams in adult care and other departments within Bristol City Council. If you are enthusiastic about vulnerable citizens in Bristol, we can offer training/supported supervision and benefits in terms of pensions and flexible working.
