Chc Assessor

The Individual Commissioning Team is responsible for the assessing, commissioning and case management for individuals with complex health needs. Following a workforce and operating model redesign an exciting opportunity has arisen for us to recruit 5 CHC assessors.

As an assessor you will provide the initial point of contact for the individual commissioning team, you will meet, their families and be required to produce a thorough and accurate assessment of thir health can social care needs. You will have to be able to make a holistic positive difference to the lives and experiences of those you assess. You will ned to be fully competent in the National Framework for CHC and FNC and operate within local and national requirements.

The role will support the Associate Director and Service Manager with ensuring that statutory responsibilities are met.

Hours of work are **Monday to Friday 09:00 - 17:00
