Clinical Navigator

The primary responsibility of the Clinical Navigator is to identify the most clinically appropriate pathway for ambulatory patients on arrival at the front door of the Emergency Department. This Clinical Navigator has significant experience and knowledge gained from working within an acute setting. Allowing rapid assessment and recognition of deteriorating and unwell adults and the ability to prioritise and expedite care as required.

They will be responsible for oversight of the ambulatory Emergency Department & waiting area. This will include identification and escalation to the Acute Floor Management team regarding capacity, safety, flow issues, waiting times and Clinical resource.

Engagement with patients and relatives play an extensive part in this role, the ability to communicate effectively in what can be a busy and pressurised environment is essential. The navigator plays a pivotal role in communicating between key personnel and departments throughout the trust and wider Health care system to develop good interprofessional, collaborative relationships.
