**Discharge Coordinator**

If you an experienced Administrator complete who can work accurately and in a timely manner, managing all correspondence of the outcomes of Clinical appointments then we could have just the role for you.

We are looking for a Discharge Coordinator to join our welcoming and diverse team based at our Plymouth Hospital

**About the role**

The main scope of the role is to insure delivery of a high standard of care to our patients on discharge and to cover the transition between the inpatient hospital setting and home.

The aim of the role is to improve the patient experience from admission to timely discharge from hospital, ensuring all health and social care needs of the patient are met, to ensure all patients and carers are given information relating to their treatment. This will include:

- Expected length of stay, Ward round information and Discharge planning.

**About You**:
**Who we're looking for**

Someone with recent and relevant experience in this kind of role, who also has excellent communication and interpersonal skills as well as the ability to work under pressure.

Just as important though is your flexibility, belief in teamwork and your ability to use your initiative, follow instructions and prioritise a busy workload.

You will also have the ability to Identify any additional social care needs that may be required on discharge, risk assess and involve the carers in discharge planning.

The Ideal applicant would have;
- Substantial administration experience
- The ability to work on your own and as part of a team
- Experience in dealing with members of the public
- A polite, friendly and approachable manner
- Warm and empathetic nature
