Team Lead for Quality Assurance and Discharges

The principle aim of the role is to support the team to improve clinical pathways, efficiencies and patient outcomes within the role's remit through an integrated system approach.

The Quality Assurance Team is made up of a discharge function as well a quality assurance function which is a collaborative and partnership approach with the BSOL acute hospitals to coordinating and planning the transfer of patients, including those with complex health and social care needs, back to their usual place of residence wherever possible following an episode in hospital.

The post-holder for this role will to ensure that system capacity and flow is managed as effectively as possible to meet the demands by working with providers to ensure best use of resources and by the commissioning of appropriate and adequate services, working with partners to identify gaps or where changes are required.
