We are seeking an enthusiastic, highly motivated Band 3 Community Support Worker to work within Torbay and South Devon Locality on a part -time 36 hour per week permanent contract. #CaretojoinusTSD

You will be joining our specialist team with the aim to visit people living in their own homes, providing support that helps them live with more independence and choice. We provide immediate, short term service over a seven day period available to adults who have an acute physical health crisis or a sudden breakdown in care.

The service aims to support safe discharges from hospital, as well as preventing admission to hospital or to a residential or nursing home.

Duties involved will include provision of personal care, clinical duties where delegated, monitoring the patient’s condition

To enable and / or assist individuals to remain in the community, to be supported and maintained safely in their own homes and thereby enhance the quality of life.

To carry out the personal tasks which are not the responsibility of a nurse but would normal expect to be undertaken by a member of the service user’s family.

Collaboration with other support workers and professionals to provide care for the service user

To provide good quality domiciliary support to the service user in their own home, promoting independence.

Assess the risk and needs of service user’s in accordance with the care plan.

The role is very varied as such you will need to liaise with other professionals to ensure the patients are supported in the most appropriate way and provide suggestions to improve support ongoing

Our service provides rapid short term intervention up to a maximum of 7 days with the aim of preventing admission to hospital or to residential / nursing home, and support discharges from hospital to facilitate earlier discharge. The services are provided by Support Workers who visit during the day from 7am till 10pm operating on a 7 day rota system, including evenings, weekends and Bank Holidays.

Torbay Hospital provides acute care for the local community and has several community hospitals, and is a hot-spot for those who are considering living by the sea or within half an hour of Dartmoor National Park and an hour to Exmoor National Park. Meanwhile, Plymouth and Exeter are also both within a short drive to enjoy the culture, maritime history and shopping experiences.

Assist and promote independent social and living skills including the provision of a range of tasks, for example: involving assistance with mobility, personal care, bathing, dressing/undressing; continence, nutrition, hydration and prompting, monitoring and administration of medication where required, in line with local policies

To ensure that patients, relatives, carers and professionals are kept fully informed of progress following intervention. Due to the diverse and complex nature of the caseload, this requires the ability to communicate complex information to meet a range of levels in understanding, and to overcome such barriers as, e.g. hearing loss, cognitive impairment, dysphasia, pain, fear, social and cultural differences

To contribute towards ensuring a safe working environment for self and others and report on situation of potential risk, working with local policies and procedures

To be aware of, and act upon, local health and safety policies, including the prompt recording of any accident, incident, or other untoward event involving self, patients or other staff

To be responsible for the use of resources in the most efficient and effective way, including the provision of short-term services
