An opportunity has arisen for an enthusiastic health practitioner to join the Homefirst Team. This position is open to a Physiotherapist or Occupational Therapist who has experience of working in the community or hospital settings and wishes to develop their assessment and rehabilitation skills.

You will be supported to develop whilst being part of a dynamic, progressive, multi-disciplinary team.The Team in Mid-Cornwall work closely with partnership organisations to support the intermediate health needs of our local population. The team are key to supporting discharges from hospital settings and preventing admission to hospital. You will be responsible for the assessment, planning and implementation of reablement and care. You will assist in the interpretation of relevant information, identifying problems and needs of a person and setting realistic goals.You will have excellent organisational skills combined with a wide knowledge base and experience of working with older persons, the community or hospital services.

**15 hours are available - a fixed term contract or a secondment opportunity is being offered.**
