You will hold a specialist case load as referred. You will be required to work flexibly, intensively and responsively to clients’ needs by generating ideas and making recommendations.

Those individuals using the service have severe, enduring and highly complex mental health needs. The aim of intervention is to deliver a high standard of care in the home, offering an alternative to potential hospital admission. The post holder also facilitates early discharge from hospital to reduce inpatient stay by monitoring and managing the risk of looking after someone who is still acutely ill in their own home.

You are required to assess need, formulate, implement and evaluate care plans for their designated specialist case load and maintain associated records taking into account all aspects of risk management

Required to develop and maintain good links and liaise with community services as well as statutory and non-statutory organisations. The post holder will also be required on a rotational basis to work as designated ward liaison practitioner. The post holder will be required to act as a designated triage person, taking referrals and assessing for suitability for Home Treatment Team. Triage role will also include gate keeping role for all acute adult inpatient beds.

The post holder will be a part of a multi-disciplinary team providing Home Treatment for people experiencing an acute mental health episode. One of the key roles is to gate-keep admissions to acute inpatient care and providing Home Treatment where appropriate. The team also seek to reduce inpatient stays by facilitating early discharge wherever appropriate.

The individuals who receive a service from the Home Treatment Team have complex and acute needs. The aim of the team is to avoid hospital admission by delivering a quality service based on individual needs of the patient in the community. The role of the post holder will also to assess for suitability for early discharge from the ward to facilitate early discharge by conducting thorough assessment of risk prior to discharge.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The post holder will follow the principals of Home Treatment team, will hold a specialised case load but will work with all clients covered by the team by taking an acute role in delivering quality, safe effective care.

The post holder will triage, assessing the appropriateness for Home Treatment team of all referrals and gate keeping of acute inpatient beds.

Assess need and develop, implement and evaluate care plans, appropriate to clients changing highly complex and often contentious issues. Lead discussions regarding own case load in Home Treatment team meetings. Attend multi-disciplinary team meetings to feed back on all Home Treatment team clients.

The post holder will carry out a thorough risk assessment during initial meeting and at subsequent meetings with service users. This assessment will inform risk management plans, which will include facilitating admissions to hospital when community alternatives are deemed to be unsuitable. This process is supported by the Trust Management Policy, which provides a framework for the worker to promote client, carer and public safety.

The post holder must ensure appropriate administration of medication by appropriately qualified staff, monitor effectiveness and any side effects of medication. Support the client to fully understand their treatment options and make informed choices. The post holder is required to monitor the physical condition of client.

Ensure Mental Health Measure process meets client’s needs in the most holistic appropriate manner. This is to be done by ensuring effective liaison with family, carers, statutory and voluntary agencies - i.e. Benefits Agency, GP’s, Chiropody, Police, Children’s Services, Housing, courts, leisure services, employment agencies, to help maintain client’s community tenure.
