Healthcare Support Worker

**JOB PURPOSE**:
To provide support, and spend therapeutic time with an allocated group of service users with mental health needs within a secure setting.
To promote service user’s recovery through the delivery of high quality, outcome measured care and evidence based interventions.
To assist Registered Professionals in the implementation and evaluation of individualised care plans.
You will be responsible for the provision of planned recovery focused
care working in partnership with service users in a secure setting.
You will be responsible for working collaboratively with other Trust
services to provide seamless care for service users on defined care pathways.
You will be responsible for working in partnership with agencies
external to the Trust supporting service user’s access to services in the
independent and non-statutory sector.

**KEY DUTIES AND RESPONSIBILITIES**:
1. Undertake shift working on 24 hour rotational basis over 7 days/nights
2. Communicate with service users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including information technology.
3. Regularly report and effectively communicate service users’ health status and care aims in care records and verbal reports to Registered Professional Staff members and other members of the multi-disciplinary team.
4. Relate appropriately with service users, carers, colleagues and others at all times maintaining professional boundaries.
5. At all times ensure that own actions, support the equality, diversity, rights and responsibilities of individuals.
6. Identify, minimise and report risks of danger, harm and abuse to senior colleagues.
7. Maintain a safe and secure environment at all times for the protection of self and others.
8. Working in secure care requires knowledge of “See Think Act Document” - Relational security.
9. At all times minimise risks to self by undertaking safe working practices.
10. At all times work within agreed and established Trust policy and operational procedures.
11. Work with other Trust services to promote seamless service user focused care, admission, transfer and discharge on identified care pathways.
12. Undertake delivery of planned therapeutic activities/interventions with an identified group of service users as an element of the care programme approach.
13. Work in partnership with service users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
14. Regularly support service users to access a range of activities and services to maintain their independence and promote their recovery.
15. Regularly support Registered Professionals and service users during activities to promote their physical and mental wellbeing.
16. Assist service users in meeting their personal care needs.
17. Provide information, support and guidance to carers, friends and relatives of service users.
18. At all times take responsibility for own personal and professional development and support the development of colleagues, students, trainees and volunteers.
19. Regularly access and make use of supervision. Ward Manager Registered Professional Staff Healthcare Support Worker Assistant Practitioner (Some Services only).
20. Regularly participate in activities focused on the delivery of a high quality customer focused service.
21. Undertake activities to, and make suggestions about how to improve services.
