Main Duties and Responsibilities 1. Communication and relationships Develop close working links with partner agencies, promoting joint working in a co-ordinated manner. Contribute to the planning, delivery and monitoring of interventions and/or treatments. Involve service users in care decisions and gain consent for specific decisions To report back to the care manager or another care manager in the team any concerns after each visit.
To update all the relevant personalised documentation under the guidance of the case load holder care manager so that resource decisions can be made. To always maintain service user confidentiality. To promote NHS SYICB when dealing with members of the public or caring for service users by ensuring all your interactions and communications promote a professional image, e.g., answering the telephone in keeping with Standards. 2.
Analytical and judgement To attend and contribute to regular staff team meetings, full CHC team meetings and to attend reviews as appropriate as directed by a case manager To demonstrate honesty and integrity in all situations, working conscientiously to a high standard. To promote a positive approach to all aspects of your role. To be flexible to the changing service users needs and service development 3. Planning and organisation To take responsibility for and fulfil your work plan objectives, being willing to learn new skills and new practices.
4. Physical Skills Deliver against organisational objectives, achieving quality outcomes, prioritising own workload, and working to tight deadlines 5. Patient/Client Care To visit individuals in a variety of care settings as well as within the individuals own home. Communicates with clients who have barriers to understanding and acceptance.
To work alongside service users and their family carers ensuring that the support that is in place is meeting the persons needs 6. Policy and Service Development To uphold NHS SYICB current Policies and Procedures. To uphold all National and Local Policies and Procedures, and ensure you are familiar with them e.g., Safeguarding, Mental Health Act, Mental Capacity Act guidance. To work to outcomes determined nationally following papers and reports outlining current best practices and standards and as directed by senior staff.
7. Finance and Physical Resources Deliver against organisational objectives, achieving quality outcomes, prioritising own workload, and working to tight deadlines Continually strive for delivering project / function outcomes, value for money and greater efficiency Contribute to the financial delivery of the service ensuring its cost effective and delivered on time 8. Human Resources To have and participate fully in an annual personal development plan and performance development review, identifying gaps in knowledge and training, ensuring you are up to date with your practice to fulfil effectively your PDR profile outline. To ensure you maintain safe working practices and attend all annual or statutory training as directed by the senior nurse, including fire safety, moving and handling and basic first aid/life support.
Assist with maintaining the welfare and safety of the service users and staff with due regard to accident reporting and the Health and Safety at Work Act. To participate in regular 1:1s with your supervisor to discuss your work performance. 9. Information Resources To be competent in the use of system one for management of patient records and information and ensure that each contact with a person or their family is documented accurately.
10. Research and Development Actively support and contribute to the development of key performance indicators for the successful assessment of performance. Contribute to ensuring there are processes in place for spreading and sharing learning and outcomes. 11.
Freedom to Act Contribute to referrals for individuals to enable clinicians to complete relevant assessments e.g., health screening, and specialist assessments. Participate in identifying clinical risks, and the evaluation of care. 12. Physical Effort Be able to work remotely and office based.
Must have a driving licence and able to travel to undertake role. Will need to adapt to environment this be care home, patients own home to be able to undertake assessment and role. 13. Mental Effort Ability to work in a busy environment.
Demonstrate adaptability, flexibility and ability to cope with uncertainty and change. Change workload and task if needed and to prioritise task, work that may come in to ensure patient safety. 14. Emotional Effort Ability to empathise and demonstrate compassion Support patient, relatives, carers and colleagues/ MDTs during difficult situation and time of their lives Support the End of life / Fast track service Listen and build relationships with patients, families, and carers.
