Working within Eden’s GP practices and working closely with the wider ICC team, the Senior Living Well Coach (SLWC) will work within the Wellbeing Service to identify and provide assessment for individuals with Long Term Conditions (LTCs) and associated psycho-social needs. The wider aim of the Wellbeing Service is to engage patients who experience such difficulties by supporting them to set personalised goals and find/access services and resources that best suit their needs and circumstances.

The Senior LWC will have skills in motivational interviewing and appropriate psychological care skills in line with core frameworks, including Cognitive Behavioral Therapy CBT, acceptance & commitment therapy (ACT), and mindfulness. Training and on-going clinical supervision will be provided by the Physical Health and Rehabilitation Psychology Department, within the North Cumbria Integrated Care NHS Foundation Trust (NCIC).

The Senior LWC will supervise a team of Living Well Coaches and will coordinate holistic assessments of need. This will include use of identified clinical assessment tools in order to facilitate effective, patient centred care planning and make onward referrals where appropriate. The LWC will work closely with the Community Teams as part of a multi-disciplinary team (MDT) approach to care planning and provision.

The ICC practices have identified that the management of patients with ‘non-medical’ and low level psycho-social needs present a significant call on GPs' time and, in many cases, the needs of such patients can be better met by another service, or by a coordinated approach to care which accesses multiple services. GPs recognise that they are not always in the best position to provide a holistic assessment of need, in order to develop a comprehensive Care Plan, which includes goal setting and review. The Role of the Wellbeing Service will be to support the “non-medical” and psycho-social needs of the individual patient. The basic criteria for access to the wellbeing includes patients who experience the effects of Long Term Health Conditions (including Frailty) and associated psycho-social difficulties.

Requests to access the Wellbeing Service will be made mainly from within practices but also by other members of the ICC team through the ICC Hub. GPs, practice staff or members of the ICC team identify patients who would benefit from the service and make a request via identified referral routes.
