Clinical Lead

Are you a nurse clinical lead with a keen interest in community care? Would you like to be part of the team working in partnership with colleagues and key stakeholders to lead the development of the first newly forming Community Response Hub within the Rural Alliance. We are looking to recruit a Clinical Lead - Community Nurse, working full time 37.5 hours per week (Weekends included and working day between 8am and 5pm included) to join our Community Response Hub.
The post holder will be a clinical resource, providing a visible clinical profile. You will have advisory skills, leadership and clinical oversight of the clinical caseload. You will work collaboratively with the wider health and social care team to meet the needs of patients, ensuring people receive the right care at the right time and in the right place. Reinforcing the concept of a more streamlined patient journey, with mínimal avoidable delays, and an emphasis on people achieving independence whilst improving the quality of their care.
You will be committed to working with our local populations in order to facilitate and support the success of the community response hub in facilitating peoples discharge home at the earliest opportunity. The successful applicant will have a proven ability to clinically lead. Ideally you will have experience in implementing and sustain changes and ability to work collaboratively and bring people together.
- To provide direction within the caseload in consideration of the impact
of current health and social care agendas and local initiatives
- To focus on the delivery of person centred care
- To provide effective, evidence-based care to a defined population
through case load management and monitoring of care standards
- Provide leadership including clinical supervision to team members and provide support to the Team managerial lead.
**We offer a relocation package of up to £8,000**
The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
