**What's the job?**

The Humberside Reconnect Service is a care after custody service that seeks to improve the care of people leaving prison by offering our service and support pre-release, and continued support in the community post release. The post holder will have a keen interest in health and justice and a passion for supporting people as they are released from prison and will work as part of a team and liaise closely with Criminal Justice Partners to work with individuals who have been referred.

The role will include supporting service users in the wider Liaison and Diversion service to ensure a seamless pathway.
