Care Coordinator- Learning Disabilities

Ripon Community Link is a charity in Ripon supporting young people and adults who have a learning disability. Our site, at Ripon Walled Garden is situated just on the outskirts of Ripon and is a beautiful historic walled garden, woodland and orchard. The garden is open to the public to enjoy and we have a nursery sales area and Garden Cafe.
**Responsibilities**:
1. To lead and co-ordinate the day to day provision of services across the organisation, working closely with the Social Care Manager and ensuring adequate staffing levels/cover is in place making any temporary adaptations to deliver safe and meaningful sessions to members that enable them to reach their targets.
2. To ensure staff cover due to staff absences, (at times with little notice) in Member sessions. To also cover staff absences, (at times with little notice) in Member sessions and be able to work from the session plan or have a contingency session they can deliver. To support the personal care needs of Members as necessary, to include feeding or physical support if required.
3. To ensure the smooth running of the buildings used by Members ensuring they are clean, tidy and safe. Carry out the designated Health and Safety checks as requested by the senior leadership team. Report any concerns to HMS and follow the protocols required to fix/repair or replace items.
4. On occasion take responsibility for planning and delivery of Member sessions normally in the café and retail area.
5. Supporting Members to learn basic skills in catering and retail areas including but not restricted to interacting, communicating, assisting, taking orders, serving, clearing tables and helping customers in the café and shop, stock management.
6. Serving customers in the café and retail areas.
7. To work with senior managers to plan and organise the delivery of activities as part of the varied but planned timetable. To monitor session content and ensure staff are recording Member target progression. To advise Support Assistants in delivery of the sessions where required and evaluate each session to identify the achievements against targets and any changes required to increase the impact and outcomes for Members.
8. To support the planning and delivery of Member Planning meetings, helping Members set theirargets annually, recording the information and ensuring this is shared with the relevant staff. Ensure Member records are kept up to date and relevant and that staff are made aware of any changes.
9. Supporting staff to identify risks and ensure staff supporting Members complete a written risk assessment. Revising and adapting risk assessments as required and sharing these for approval with SCM or HMS.
10.To be the first point of contact on a day to basis to ensure the smooth running of the Member service. Liaising with parents/carers, transport etc and supporting staff with their queries offering guidance and advice ass and when needed, escalating questions or concerns to HMS or SCM.
11.To act as Key Worker for allocated members and liaise with the Members Link Person to monitor their progress or concerns. Research and share information to the team, for example a health condition, and how best to support the Member.
12.To recruit, line manage and motivate Learning Disability Support workers and those with additional responsibilities, conducting regular contact meetings, supervision sessions and annual appraisals, reviewing and supporting their training needs to maximise their contribution to the service and ensuring safe and effective practice. To support in the induction of new staff, volunteers or work experience students. To directly support work experience students when they are with us and be their first point of contact for support. To contribute to the training of new and existing staff and volunteers, through comprehensive induction programmes or focussed training sessions.
13.To encourage the involvement of volunteers in Members sessions, providing support and guidance to them make the maximum contribution to the sessions safely and appropriately.
14.Conduct staff meetings as and when needed, liaising with other members of the team as necessary.
15.To hold a full cleaning driving licence and a be prepared to drive Ripon Community Link vehicles.
16.To assist, when needed, the SCM to showcase the service we offer to potential new Members.
17.To assist the Social Care Manager and Head of Member Services in developing strategies to deal with any crisis situations involving Members, including identifying potential problems contributing to meetings with other agencies, and carers concerning the development and support to Members.
18.To adhere to organisational financial policies and procedures including the safe handling of cash, ensuring that all expenditure is within budget and contributing to the robust management of expenditure and income within the organisation.
