123029 Transport Co-ordinator

The Capacity & Flow Team are a dedicated team who provide skilled and professional support in directing patient flow across the acute division.

The Transport Hub Co-coordinator supports patient flow out of hospital for all in-patient discharges and non emergency transfers to community hospitals. This is an exciting role for an individual to be part of the Capacity and Flow site team to facilitate patient flow.

We are looking to appoint an individual to co-ordinate and arrange transport by triaging a number of transport options best suited for safe transfer for patients and to support and ensure wards have patients ready timely for transport and transfer to the Discharge Lounge where appropriate.

You will liaise closely with the ambulance service to co-ordinate and utilise available transport, receive and provide essential information and advise staff on transport options to meet the patients needs and direct book patient transport with ambulance control either by phone or direct computerized access. You will play a key role in driving the use of technology within the transport hub and designing local processes for direct digital bookings.

You will collect and input data on paper and computerised systems, including Trakcare, Microsoft Excel and Word to maintain detailed records including direct booking with ambulance control, act as a point of enquiry for multiple enquiries from staff by telephone.

The role requires you to liaise multiple times daily with wards to ensure updated information is given and received to co-ordinate a safe discharge or transfer.

You will ensure that staff rostering is completed for the Capacity & Flow Team, with input onto SSTS, maintain staff training records, compile daily reports of Transport activity and provide general team administration support. The role requires autonomous working with a high level of communication skills.

You will maintain high standards of quality and patient centered care and communicate effectively to facilitate capacity and flow.
