**SEND Practitioner**:
Reporting to: Head of Early Years/Nursery Manager.
Duration: 21 hours per week, fixed-term contract (funded role for 3 years).

**Salary**: £19,000.00 pro-rata for 21 hours or 0.6 FTE.
Location: Plumstead nursery base (though across all HSG centres).
Staffing: None.

**Job Purpose**

To complement the work of the Early years team, SEND and Inclusion service by taking responsibility for SEND children and
families attending Home-Start Greenwich services across all sites. This includes supporting agreed learning activities for groups
or specific individual children who have complex or special educational needs.
The primary focus is to support the development and inclusion of children through:
1. Direct work with individuals, groups, and families.
2. Coaching, mentoring and capacity building with education providers.

**Role and responsibilities**
- Plan, prepare and deliver activities for individuals and groups of children under the professional direction and

supervision of Head of Early Years and external SEND and inclusion specialist differentiating and adapting learning
programmes to meet the needs of individual children.
- To work with children and families who have extremely complex or additional needs and in some instances, life limiting

conditions.
- Supply specialist support with direction and guidance from SEND and inclusion specialist, allowing children to access

The Early Years foundation statuary framework.
- Coaching/ assisting staff to implement SEND support plans.
- Contribute to and assist in planning, organising, and implementing individual children’s developmental plans including

attendance and contributing to reviews.
- Establish and maintain relationships with families, carers, and other professionals.
- Plan and deliver SEND group activities e.g., Stay and play sessions for families.
- Be responsible for preparation, maintenance, and monitoring of stocks of resources and materials.
- Plan and evaluate specialist learning activities with staff.
- Support colleagues in report writing records and providing focussed personalised provision as required.

**Skills and experience**
- Experience in Speech & language & communicational skills with children 0-5 years.
- Comfortable working in partnership within the broader Home-Start management, other organisational disciplines,

external education providers, and agencies.
- Enhanced DBS clearance and suitable Disqualification by Association record.
