**Job description**:Arun & Chichester Citizens Advice deliver a Social Prescribing service for Angmering, Coppice and Fitzalan Surgeries in the Littlehampton district and we are looking to hire a dedicated Care Coordinator to help support clients access the services they need.

Our service supports clients to improve their wellbeing through linking them with services, such as advice agencies, hobby/interest groups, gaining employment/volunteering and mental health support. We support anyone aged 18+ but in particular have many clients with chronic pain conditions or living with poor mental health.

A Care Coordinator's responsibilities include consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care. The approach is always person-centred, so you’ll be required to adapt your delivery depending on the client’s needs.

You’ll learn how to use our systems and NHS systems, including our case management system, to work closely with the Social Prescribing team and other health professionals to create care plans and ensure patients have access to health services.

Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their health challenges.

- A good understanding of the aims and practice of Social Prescribing

**Job Types**: Part-time, Fixed term contract
Contract length: 12 months
Part-time hours: 22 per week

**Salary**: £12.00-£13.00 per hour
