Assistant Practitioner

We have a new and exciting opportunity to recruit 2 highly motivated Band 4 Assistant Nurse Practitioners, to join our well-established team of specialist nurses in the Community Heart Failure service at Liverpool University Hospital NHS Foundation Trust.
The team provide rapid assessment, hospital admission avoidance, timely optimisation of evidence based treatment and therapies for patients, delivering a cohesive management programme for patients in line with local and national guidelines)
The role will be working across acute, community and virtual ward settings. The service will operate 7 days per week.
The role requires flexibility, knowledge of heart failure, clinical skills, team working within an evolving proactive, dynamic and committed multidisciplinary community Heart failure Team.
These is a fixed term position
Work collaboratively within the team/with other agencies deliver local and national health priorities and objectives.
Accountable for own clinical practice. Deliver care in line with national quality standards and Trust policy.
Work within the skills and competencies expected of the role. Act as an advocate/champion for clients.
Support clinical staff managing long term conditions. Organise/ facilitate groups/or individuals to develop self-care skills.
Identify need for and initiate referrals to internal/external partners, within agreed parameters.
Develop ‘person specific goals’ with individuals, carers and colleagues.
Enable individuals to develop the necessary skills to improve own health and lifestyle.Undertake risk assessment in line with relevant guidelines.
Undertake health promotion activities as Undertake clinical tasks, following patient specific direction.
Communicate effectively with patients and carers. Participate in individual clinical supervision.
Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation policies and procedures.Provide information as requested.Undertake all relevant mandatory training.
Develop and maintain own knowledge and skills through the use of competency frameworks and continuous professional development (CPD) and personal development process (PDP).
Provide cover for and undertake duties of absent colleagues. follow all the policies and procedures of the organisation
Liverpool University Hospitals NHS Foundation Trust was created on 1 October 2019 following the merger of two adult acute Trusts, Aintree University Hospital NHS Foundation Trust and the Royal Liverpool and Broadgreen University Hospitals NHS Trust.
