**Job Purpose**

The aim of the Care Co-ordinator is to coordinate the delivery of a safe, well planned homecare service based within the Harrogate and Knaresborough locality ensuring continuity of care planning and work allocation to customer visits and carer allocation. You will be responsible for producing staff rosters using our Birdie scheduling software system ensuring the safe, efficient planning of carers work allocation. The onboarding of new customers and new carers of our service ensuring the care planning system runs effectively and efficiently and that data inputted to create schedules is detailed and regularly reviewed.

Continued Care Ltd is an award winning and ambitious, forward-thinking organisation and you must be committed to developing and enhancing the services we provide including the onboarding of new carers and new customer referrals. In turn we will ensure all training and development is offered to you so you can develop your knowledge and skills. Excellent interpersonal skills will be required to liaise effectively with our commissioners and partners including social services, health, and private customers. A passion for delivering a quality and responsive service is key as is team working and attention to detail. The ability to problem solves and prioritise work is essential as is a customer focused approach through developing and maintaining effective partnership with our customers, staff, and commissioners. The care coordinator role will include care delivery if needed and you will be part of our on call emergency out of hours duty team.

**Duties & Responsibilities**
- Ensure correct input of data of service provision and delivery inputted into the Rostering systems.
- Ensure all data is kept up to date and regularly reviewed.
- Answering and taking new referrals or changes in planning of Service Users details over the telephone and entering these immediately onto the relevant IT and communication systems.
- To lead by example, be professional, helpful and informative to prospective Service Users, relatives, Health Professionals and staff members.
- To demonstrate effective interpersonal and business liaison skills.
- Liaising with managers, senior support workers and carers to ensure they have accurate and detailed information to deliver a quality service.
- Be part of our emergency on call to Carers/ Service Users on a rotational basis.
- When short staffed to work in the community to ensure all jobs are covered.
- Maintain confidentially in line with the Data Protection Act and GDPR and ensure safe storage of information/data.
- Monitoring of service users that are in hospital/ respite or service amendments have changed.
- Responding to our referral inbox and inputting new customers and carers on to our systems.
- Ensuring our systems are kept up to date and the inputting of staff holidays and sickness.
- Ensuring all visits are covered and allocated in the event of staff sickness or unexpected situations /events.
- Report concerns or issues directly to Management and escalate.
- Working to include every other weekend.
- Hold responsibility for rota planning and liaising with carers prior to their care calls and managing client visits.
- Support new starters to Continued Care, by managing their rota and listening to their feedback as part of the Induction process
- Take part in and assist with the recruitment of new Carers to Continued Care.

**Administration**
- Report to management any staff concerns, Service User complaints and actions taken.
- Successfully input carers holiday into the Rostering system. Ensuring there is appropriate rota cover in place for these.
- Ability to document and communicate effectively to the wider team using Microsoft teams.
- Over sea and coordinate care creation and visit allocation imputing correct and comprehensive data into our Birdie software and scheduling system.

**Personnel**
- Attendance in staff team meetings.
- Correct reporting of any incidents/accidents in accordance with Health & Safety/COSHH requirements to management and escalating concerns.
- To comply with all Continued Care policies and procedures including working time directive.
- Keep up to date with system changes on Birdie (Care management system)
- Awareness of Care Quality Commission regulations and KLOE’s.
- Awareness of safeguarding concerns recording and reporting processes.
