This role will sit within the Targeted Youth Support service and will involve directly engaging with young people who are currently receiving support from Somerset Foundation Trust for a diagnosis of Diabetes, by delivering youth work services and support them in a strength focused framework. Work completed will be organic, may include unsociable hours, and participation by young people will be voluntary. The youth workers will need to be:

- Qualified in Youth and Community Work (Level 3 or above, or equivalent in a similar discipline with relevant experience)
- Experienced in working with young people within a variety of settings
- Able to demonstrate effective partnership working
- Have a comprehensive understanding of safeguarding and supporting young people with complex backgrounds

Using a youth work approach, the Diabetes Community Youth Worker will provide, 1:1 support for young people within the family home, schools or clinical and community settings, and facilitate small groups for their caseload, whilst being led by the diabetes clinical team, by:

- Providing 1:1 mentoring and positive activities for young people
- Advocating for the young people and their family within professional settings
- Signposting to relevant services within Young Somerset and externally
- Ensuring vigilance whilst maintaining Young Somerset’s safeguarding principles

**Salary**: £25,991 - £27,041

**Hours**: 37hrs per week
