**Job title: Physical Activity Link Worker (Childrens Healthy Lifestyle Practitioner)**

**Location: Manchester (Office base in Eccles)**

**Hours: 37.5hrs (will include evening and weekend work)**

**Reporting To: Service Lead**

**Key Relationships: Service Lead, Contract Manager, Client Services Team, Local Partners and Stakeholders.**

**Closing Date: 9th February 2023**

**The Role**:
To work with children and young people (including their families) to make positive healthy lifestyle changes with a focus on including increasing physical activity levels. To signpost/link and support clients into engaging in local physical activity offers. The post holder will support clients to set realistic goals when it comes to changing behaviour and use an asset-based approach working alongside local community initiatives to assist, with the longer-term goal of reducing BMI and potential future risk to health.

**Key responsibilities: -**
- To undertake person-centred holistic assessments (including anthropometric measurements) with individual service users and co-design healthy lifestyle action plans as part of a physical activity referral, identifying support needs to ensure maximum engagement in improving health and well-being.
- Support individuals to set realistic goals in terms of changing behaviour and achieving positive healthy outcomes.
- Engage with a multi-agency network of organisations to work collaboratively to support the delivery physical activity programmes for children/ young people.
- Connect/signpost and refer clients to one or more physical activity programme/service.
- Consider individual needs of clients and their families and provide information and advice to direct them to the right services/support to support them to engage effectively.
- Adhere to the child protection procedures and local safeguarding policies.
- To maintain excellent records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.
- Assist in the collection of monitoring data in addition to collating feedback from service users in order to support the development of the programme.
- To record, monitor and, with the team lead of the service, actively improve on and contribute to the achievement of all local KPIs.
- To facilitate interactive and engaging lifestyle educational sessions, following the MoreLife curriculum, to participants on the programme.
