**We are currently looking for Paramedics with 5+ years experience for a Primary Care Network in Hackney, North London**

**1 - 2 days per week, Thursdays and Fridays**

**Paramedics Enhancing Primary Care**

The significant contributions that experienced Paramedics can make to Primary Care are now widely recognised. These contributions include providing crucial support to patients in the community and helping to reduce avoidable hospital admissions. Paramedics working in the community can reduce hospital admissions with 70-80%.

We are actively seeking enthusiastic Paramedics who are interested in becoming part of a local initiative in North London within the realm of Primary Care. Our expanding array of exciting and diverse projects offers a unique opportunity for those seeking a forward-thinking alternative from the traditional rush of 'blue light' emergencies.

The Role will require a commitment to register on a First Contact Practitioner pathway (FCP), which we will fully support you in. You will collaborate closely with GPs and other Healthcare Professionals, conducting patient assessments, assisting with care plans, making referrals to community teams, updating records, ensuring patient safety, administering vaccinations, and managing patients within the appropriate care setting.

The work a FCP Paramedic undertakes in a PCN varies from practice to practice. Possible tasks include:

- Acute and chronic disease management home visiting service
- Care home ward rounds
- One the day clinics for acute conditions
- Advance care planning conversations
- Long term condition reviews
- COPD/ asthma reviews
- ECGs, BP checks and phlbotomy
- Learning disability, dementia health checks
- Visiting house-bound or vulnerable patients
- And much more

**Requirements**:

- **Essential**:_
- HCPC registered Paramedic since 2019 or earlier.
- Interested in dealing with non-acute patient care in the community.
- Willingness to complete essential development and portfolio work.
- **Desirable**:_
- Urgent and Primary Care HEE level 6 education

Pay: £25.50-£35.95 per hour

Expected hours: 7 - 18 per week
