**Children and Young People (CYP) Social Prescribing Link Worker**

Telford Mind have been contracted to provide a part time Social Prescribing post for 12 months to support the Wellness While Waiting programme (with UCL, Anne Freud & MPFT) as part of the Personalised Care work for STW.

The project will trial offering immediate social prescribing treatments to children and young people referred to services for moderate mental health conditions, such as anxiety, depression or PTSD. Currently, many children and young people face long waits for mental health services, during which time their mental health often deteriorates. The social prescribing pathway will be part of a research project and will be offered as children and young people wait for their treatments. This project will cover Telford and Shropshire.

**Contract**:
Fixed Term for 12 months.

**Hours**:
20 hours per week, Monday - Friday between 9.00 am - 5.00 pm (flexible working is required to suit the needs of the service).

**Place of Work**:
You will be based at Telford Mind (TF7 5EE) but will be required to work Telford and Shropshire.

£13,863.00 p/a

**Job Purpose**:
To work with children and young people across Telford and Shropshire, aged 11-18, to empower them to gain control of their health and wellbeing utilising person-centred and asset-based approaches.

You will be required to develop and deliver a Children and Young People’s (CYP) Social Prescribing service, ensuring that the service supports patients to take control of their wellbeing and live happier, healthier lives. The Children and Young People’s link worker will support people with their emotional health and wellbeing needs and supporting them to access and engage with community support.

You will be required to provide support to patients face-to-face, via telephone and video call. You will carry out an initial assessment to help them identify their health, social and well-being goals. You will then work closely with the children and young people to develop a personal plan of desired outcomes and actions referring them to voluntary sector services, community groups, and activity clubs and, where relevant, public sector services in order to achieve these outcomes and actions. You will develop a strong and coherent knowledge of the local voluntary and community sector, thus ensuring that the young people are referred appropriately with your support. Additionally, you will build links with voluntary sector organisations, community groups, health and social care partners engaging and acting as an advocate for the project, building relationships for the future referral of patients.

The link worker will work collaboratively with BeeU/CAMHs, taking identified referrals for young people.

You will be working across Telford and Shropshire and will be required to develop and deliver the service at times which are appropriate for access by the children and young people, which will include some evenings and weekends.

As social prescribing is continuously evolving, you will also be feeding back what works well in order to develop models of good practice. You will work closely with BeeU/CAMHs and clinicians as well as multi-disciplinary teams to achieve positive, sustainable outcomes for Children and Young People.

You will need experience and qualifications relevant to the role but also a desire to make a difference.

**A full driving licence and access to your own vehicle is essential for this role.**

**Main Duties**

**Development**
- Raise awareness and understanding of the CYP Social Prescribing function and link worker role within the CYP BeeU/CAMHs service.
- Build relationships with key staff within BeeU/CAMHs, attending relevant meetings, becoming part of the wider network team, giving information and feedback on CYP social prescribing
- Work in partnership with BeeU/CAMHs to understand the children and young people's needs, using local knowledge and patient data to target specific cohorts of people and ensure service data is captured.
- ProvideBeeU/CAMHs and other referral agencies with regular updates about social prescribing, including information to encourage appropriate referrals.
- Seek regular feedback about the quality of service and impact of social prescribing.

**Delivery**
- To listen and respond to the voices of children and young people and structure support to reflect this and ensure their needs are met.
- Provide personalised one-to-one support for children and young people aged 11-18.
- Develop trusting, professional relationships by giving young people time and focus on “what matters to me”, taking a holis
