To take responsibility for leading on patient flow within Cornwall Partnership Foundation Trust within your designated locality. The purpose of the role is to assist in developing and establishing new robust systems and processes to deliver safe and effective care with efficient use of resources both within Cornwall Partnership Trust and where Cornwall Partnership Trust interfaces with key partner organisations within the local health community ensuring the right patient in the right place at the right time. To lead on key projects identified within the Patient Flow agenda and be instrumental in delivering on key performance indicators associated with operational management. There will be a requirement to work 7 days per week with a flexible approach to days and hours worked. The post holder under the direction of the Head of Operations will be expected to work across the boundaries of patient care with Health and Social Care colleagues.
- Working with clinical teams, onward care teams and site co-ordinators within the acute trusts to assist with timely and appropriate transfer of patients e.g. to community hospital, home setting etc.
- Liaising and working closely with all community teams e.g. community hospital Ward Managers and Matrons, Locality Directors and District Nurse leads to ensure timely and seamless transfer of patient care to community services.
- Engaging and developing networks within local communities to include private, volunteer, social care and health in order to enable localities to optimise opportunities to improve the health, wellbeing and independence of people at risk ensuring that the right patient is in the right place at the right time.
