Job Description/Person Specification

We are looking for a Team Leader who has passion and vision for the delivery of excellent person-centred services, leading a small team working to support a young man with learning disabilities and other complex needs. If you feel you have the relevant experience, commitment and drive to lead a skilled team, please get in touch.

The post holder will work to deliver services ‘hands on’ as the leader of the team helping this individual to lead a fulfilling life within their home and community. You will work to shape the service and to ensure high quality outcomes are achieved across Supported Living Services as a part of the wider management team.

You will deliver services on rota, and the management task is to lead the team delivering high quality outcomes to help an individual with complex needs to live a fulfilled life. You will be responsible for ensuring that agreed outcomes from individual person-centred plans are met and to maintain excellent working relationships with the family, key professionals and the wider community.
