This is a highly responsible position which listens and responds to the needs and aspirations of individual customers and carers after they have received a formal diagnosis of dementia within in the communities of Gateshead. To communicate with clients in a comforting person centred way, creating a relationship with the client to guide them through their journey after diagnosis. To help make sure all information provided is up to date, and to direct clients and aid with contact to the appropriate services. Demonstrate an understanding and awareness of the policies and procedures of Age UK Gateshead, ensuring that they are maintained at all times.

MAIN DUTIES AND RESPONSIBILITIES

1) Contacting new and existing individuals from the dementia register.

2) To ensure a personalised approach to each individual, and provide quality up to date guidance to meet their specific needs

3) To keep up to date with available services that will benefit your clients. JOB DESCRIPTION Community Dementia Support Worker

4) Complete home visits, risk assessments, holistic assessments and aid the individual in improving quality of life.

5) Help in the promotion of cognitive and physical activities for customers through outings, conversation and other activities such as reading, writing, hobbies and recreational interests, reminiscence.

6) To work with colleges, supervisors and PCN leads / GP Surgery’s

7) To read and record accurate information in all instances i:e documenting conversations.

8) To observe confidentiality at all times.

9) To participate in training courses and activities as required

10) To provide cover for other Age UK Gateshead support worker provision as and when required.

11) To travel from client to client to deliver a service as well as operating from a hub location.

12) To carry out other reasonable duties at the request of the Manager or CEO.

**PERSON SPECIFICATION PERSON SPECIFICATION ESSENTIAL DESIRABLE EDUCATION AND TRAINING **Good basic education

Full driving licence

Level 2 Health and Safety Certificate Moving & Handling Certificate Risk Assessment Certificate L2 in Health and Social Care

**EXPERIENCE**

At least 2 years’ experience of working with adults within a social care setting.

Experience of working within a mixed staff team Experience of working with older people.

Experience with dementia care.

Experience with Health and social care.

**KNOWLEDGE AND SKILLS**

Numeracy and literacy skills.

Daily report writing.

Able to work as part of a team as well as unsupervised

Sensitivity to customer and carer requirements

Ability to be tolerant and tactful

A good listener and communicator

Full driving license
