Are you a confident practitioner? Why not join LCC and play a key role in keeping the residents of Leicester City safe, healthy, and well. We are currently recruiting for an organised Health and Social Care Co-ordinator to join our Adult Mental Health Services.

The Adult Mental Health Team are a dedicated team, one of our objectives is to enrich the lives of those returning home from hospital. To support them to maintain their independence and promote healthy living in a safe manner within the comfort of their own home. This could be through a variety of options including, but not limited to, interim placements, supported living, residential care etc dependant on need and risks. You’ll play an integral role in supporting with minimising unnecessary delayed hospital discharges in mental health hospital rehabilitation setting.

As a Health and Social Care Co-ordinator you’ll join a team and work in collaboration with a range of professionals and agencies including Consultant Psychiatrists, Community Psychiatric Nurses, Occupational Therapists, Housing, Social Workers, Mental Health practitioners.

You’ll be based in the Adult Mental Health division of Social Care and Education. This compromises of three adult mental teams, Central, East and West, a forensic mental health team, Approved Mental Health Professional (AMHP) team and the Deprivation of Liberty Team (Dols). The role you will be undertaking will be based in one of the adult mental health teams and you’ll be working and taking referrals across all three teams.

This position is a full time fixed term position, working 37 hours per week (for 12 months). You’ll have flexibility in hybrid working being both offices based, or working remotely from home to suit your needs. You’ll also be required to attend home visits which could be in a variety of settings, as well as visiting and meeting people in a hospital setting.

**What you’ll be doing**

Working and adhering to legislation and policies which include the Care Act 2014, Mental Capacity Act 2005, Equality Act 2010 you’ll undertake assessments to assess any care and support needs as well as devising strength based and person-centred support plans. In addition, you’ll liaise daily with patients, Consultant Psychiatrist and nurses to ensure that the support is in place to ensure people get to live the life they want. Through meaningful conversations with the person, you’ll support them in recognising their strengths, aims and aspirations. This will include working alongside a variety of health colleagues to identify any barriers that may prevent a timely and safe hospital discharge. Acting as a first point of contact for the individual you’ll offer support and advice, where able, and signpost to relevant agencies and organisations for additional information so the individual is fully-informed from the off-set and throughout. Not only will you ascertain the views of the individual using our service, you’ll also offer support and guidance to the person’s carers.
