**Main Duties/Key Tasks**: Reviewing medical records and producing an accurate summary of the patients medical history. Entering correct Read Code information from paper records into the practice clinical system (SystmOne) Scanning of patient related documentation and attaching scanned documents to patients healthcare records Coding clinical information from letters, discharge summaries, notes etc. Prepare medical reports and generate letters of correspondence relating to the management of patient care. Monitoring progress of notes summarising against practice targets Maintaining awareness of clinical code changes and introduction of new codes Prioritise and Complete tasks received in a timely manner.
