The resourceful Connector Coordinator will engage with Primary Care Network (PCN) colleagues, cohorts of identified patients or service personnel and community organisations that provide support across the two geographical areas. The three clinical areas of health inequalities this project will focus upon are:
- Severe Mental Illness, anxiety, depression for the under 30-year-olds - aiming to organise for an annual health check for 60% of those under 30-year-olds engaged with severe mental illness including young people
- Hypertension case finding for the 30+ year olds - to enable interventions to optimize blood pressure and minimise the risk of myocardial infarction and stroke for those 30+ year olds
- Improved continuity of maternity care amongst Armed Forces communities across Fareham and Gosport, particularly those identified with minority ethnic heritage

Connector Coordinator key tasks
The Connector Coordinator will:
- Undertake appropriate induction and training required for the role
- Review any relevant health data and any past learning provided by the PCNs
- Co-produce a role profile for small groups of Community Connector Champion volunteers (with lived experience) across the Fareham and Gosport area for the three clinical cohorts using experience from the cohorts to shape the role profile.
- Supported by PCN and GVA colleagues, coordinate the Community Connector Champion volunteer role’s promotion across the areas via a variety of relevant sources and medias
- Initially supported by GVA’s Volunteer Coordinator and others as appropriate, recruit, arrange training for and oversee, small groups of Community Connector Champions, across the two PCN areas
- Engage with appropriate PCN/Service colleagues - PCN/Practice Managers, Social Prescribers, Health Connectors, Wellbeing Coaches, Care Coordinators and community stakeholders including the Fareham and Gosport Voluntary Sector Health Forum
- Attend relevant meetings across the two PCN areas to promote and update as required
- Keep the Project Manager and Project Steering Group up-to-date re project developments, progress against the project’s Key Performance Indicators (KPIs) as identified in the SMART Project Plan - asking for support as required
- Once underway, co-produce, undertake and analyse simple, targeted participant surveys - to ascertain feedback and the project’s impact to enable service change for the better
- Engage with and learn from other local/regional Community Connectors
- Identify areas of best practice and learning as the project progresses
- Collate the required information for Core20PLUS5 Case Studies (including via Social Media) and End of Project Evaluation Report. Organise a Fareham & Gosport PCNs/Services sharing best practice/learning event - should project funding/capacity allow
- Supported by the Project Manager, ensure that the voice of the seldom heard communities is considered by the decision-makers and where practicable, acted upon.

**Job Types**: Part-time, Fixed term contract
Contract length: 12 months
Part-time hours: 21 per week
