Nottingham University Hospitals NHS Trust [NUH] is a large university teaching hospital. The demographics of the region deliver a higher rate of cancers per head of population that the England average, and this combines with adverse workforce presence in the region. Difficulties in demands on primary care, pressures on the diagnostic workforce are well described in line with the national picture, and the number of oncologists per head of catchment population is amongst the lowest in England in the East Midlands.

Nationally, around 25% of all cancers are diagnosed via an emergency secondary care presentation, a further 25% are via 2WW referrals from primary care, and up to 50% come as the result of routine work pathways via primary care. Very small numbers in specific cancer types are detected through screening programs. NUH Cancer Centre figures show to be average in these respects.

It has been the linchpin of a move to an individual and ‘_patient plus clinician first_’ approach to more complex needs. Its model involves seeing the patient in clinic face to face first to have a history taken, an examination performed by an experienced clinician and specific tests requested on a defined differential diagnosis.

Main duties of the job

Working for our organisation

The specific aim of the Geriatric Fellowship in diagnostic oncology is to gain clinical experience in the diagnosis of cancer and its differential diagnosis. Previous dedicated oncological training is not required for the post - the purpose of this clinical training is to consolidate knowledge and experience in diagnosing cancer and recognizing complications of cancer. Current and previous NSCP 2WW team members have had no previous dedicated oncological training (are medical registrars, for example with no Foundation or Core Medical exposure to Oncology) but have relied on their general medicine skills to give to the patient and expanded their knowledge base through this diagnostic oncology training experience. The Geriatric Fellowship will also facilitate targeted experience in the diagnosis and treatment of the older person with cancer.

Detailed job description and main responsibilities

NUH Cancer Centre is offering a 3-month full time rotation in Diagnostic Oncology to a medical or oncology speciality training registrar. This unique post will offer dedicated training in diagnosing cancer and recognising cancer complications. All substantive NUH training placements have GMC approved training environment. Approved diagnostic oncology educational and clinical supervisors act as clinical supervisors to the Geriatric Fellow. The Geriatric training programme maintains responsibility for ensuring that the trainee has a named educational supervisor whilst on placement in diagnostic oncology.

The diagnostic oncology supervisor should have regular meetings with the GP trainee to discuss learning needs, provide feedback and support. There must be a placement planning meeting at the start of the placement. At the end of the placement there must be a meeting to discuss and complete a clinical supervisor report. This report is reviewed by the GP trainee’s educational supervisor at their next educational supervisor meeting.
