This is an exciting opportunity to join the Renal Home Therapy Service; we are recruiting a Band 6 Home therapies dialysis Co-ordinator for 22.5 hours per week.

The role involves the education, training, and promotion of home dialysis as a treatment modality to patients and relatives to enable them to make an informed choice and dialyse independently at home.

A large part of this role can be completed working from home, but you will be required to work across the hospital-based training unit at Chorley District General Hospital as well as the community setting covering Lancashire and South Cumbria including our new Kendal home therapies training unit. Unit opening hours are 07:00-18:00 with shifts of 7:00-15:00 and 10:00-18:00 Monday to Friday. The service also provides an acute drop-in service for the home dialysis patient and an out of hours on call advice service 7 days per week.

Ideally you should have current relevant post registration experience working within the renal specialty with experience in home therapy. Being a competent car driver and owner is essential as the community setting covers a large geographical area. Flexibility is required for this role as some patients may require evening visits.

You will also be highly motivated with excellent communication, interpersonal and organisational skills and can work within a multi-disciplinary team setting.

**Duties to include but not be limited to**:

- Support the Senior Sister/Charge Nurse or Matron in ensuring the provision of excellent care with compassion for the patients, relatives and visitors who are cared for within or by the ward, department, service or team.
- Ensure a high standard of nursing is delivered by self and others based on assessment, planning, implementation and evaluation of care to meet patients’ needs in accordance with agreed local and National standards and evidence based guidelines.
- Assist and support the Manager in the safe, effective management of the area including management of personnel, facilities and the associated systems and processes.
- Act as a role model, presenting a positive image of self, team and the organisation.
- Be responsible for the delivery and co-ordination of care, through appropriate professional practice, delegation and supervision of duties carried out by junior staff.
- Set, monitor and maintain standards of care to improve the patient experience and outcome of care.
- Proactively manage risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines.
- Communicates all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care.
