Responsibilities:

- Coordinate and schedule appointments for patients
- Communicate with healthcare providers, patients, and their families to ensure proper care coordination
- Maintain accurate and up-to-date patient records
- Assist in the development and implementation of care plans
- Provide support and guidance to patients and their families regarding healthcare options and resources
- Collaborate with interdisciplinary teams to ensure continuity of care
- Monitor patient progress and make appropriate adjustments to care plans as needed
- Ensure compliance with all relevant healthcare regulations and guidelines

**Skills**:

- Strong organizational skills with the ability to prioritize tasks effectively
- Excellent communication skills, both written and verbal
- Proficient in using office software such as Microsoft Word, Excel, and Outlook
- Ability to work well in a team environment
- Attention to detail and accuracy in record keeping
- Knowledge of medical terminology is a plus

As a Care Coordinator, you will play a vital role in ensuring that patients receive the necessary care and support they need.
