Annual basic salary : £22,230 per annum (pro-rated for part-time)

Additional allowances : £1353.42 per annum (essential car user allowance)

Location: Shoreham, West Sussex

Hours: 30 hours per week

Work Pattern: Monday to Friday (no bank holidays)

United Response is looking for an empathetic and compassionate individual to join our brilliant Community Mental Health Service at the Shoreham Assessment Treatment service as a Support Worker in which you will be working within an NHS Community Mental Health Assessment and Treatment Service to support adults with severe and enduring mental health needs.

United Response is a proud member of the Pathfinder Alliance that is made up of 11 organisations that all aim to provide assessment and sign posting services to individuals with mental health needs.

**What would you be doing?**

As a Community Support Worker within the Community Mental Health Assessment and Treatment Service you would be responsible for:

- Carrying out Support as outlined by the Lead Practioners
- Following up on assessments
- Signposting individuals to other services
- Taking the time to build trust to and develop a rapport with patients.
- Maintaining contact and reporting signs of mental health relapses.
- Liaising with other organisations
- Working within multi agency teams
- Providing monthly monitoring forms on caseloads and recovery feedback
- Delivering recovery focus support for patients and recording outcome

**What we need from you**

Given the independent nature of this role we are looking for people who have the ability to use their initiative and can adapt to the person in front of you. It is vital that you are an empathetic individual with a friendly manner who understands the courage that it often requires for someone to contact our service. To be considered for this Community Support Worker post you should have 1 Years’ minimum experience working in mental health and have a full drivers licence with access to your own vehicle and a NVQ 3 in Health and Social Care or equivalent.
