Job description:
With guidance, support and supervision from the CLD or Senior Carer to be able to work autonomously:

- To assist identified individuals who have suffered a stroke or who have a neurological condition or the elderly to participate in activities that they need to or want to do.
- To assist individuals to undertake personal activities of daily living.
- To support individuals to access the community.
- To work alongside the individual’s family / carers to support them.
- To report intervention in writing using the system provided and verbally to the CLD when required.
- The role is in operation 365 days per year; therefore there are options of work pattern.

Clinical Duties:
With guidance, support and supervision from the CLD to be able to work autonomously to:

- Assist individuals to undertake personal activities of daily living.
- Assist with domestic activities of daily living.
- To assist individuals with administration of medication using the system in place e.g. blister pack
- Assist with individuals mobility indoors (and possibly outdoors)
- Enable / support the individual to access resources and participate within the community e.g. shopping, attendance at medical appointments. This will necessitate driving individuals in your car with the necessary insurance.
- Support the individual with self management e.g. orientation tasks and maintaining diary.
- Practice with elements of stroke / neuro specialist therapy to improve aspects of movement, balance and therefore function.

**Job Types**: Full-time, Part-time

**Salary**: £12,000.00-£27,000.00 per year

Expected hours: 20 - 40 per week
