**Job Summary**

The main responsibilities of the post-holder will be to read code relevant information from patient’s clinical records, hospital correspondence and other sources, into the practice computer system. Working strictly in accordance with the Document Management Protocol and other administrative protocols, this will also include inputting confidential information

**Primary Responsibilities**
- To be an active member of the Data Administration Team, undertaking ongoing training and development in document management and note summarising.
- Ensure inbound correspondence, by whatever method of receipt, is managed and flowed through to the appropriate recipient / department for action.
- Workflow and index incoming electronic correspondence, such as Out Of Hours / 111 / Electronic Discharges / A&E’s
- To scan patient correspondence to the computer clinical system and index to the correctly identified patient record, if required.
- Read code relevant information into the practice clinical system (SystmOne) and ensure follow up actions are identified and completed in a timely manner
- To keep coding and data input up to date, liaising with the Data Administration Supervisor, as necessary.
- To provide an efficient and high-quality service, by ensuring accurate data entry is recorded into the patient record
- To ensure receipt of patient GP2GP medical records and process into the clinical system.
- To summarise the medical notes of new patients to provide a computer record of their previous medical history and interventions, following the Note Summarising Protocol and practice deadlines
- To summarise ‘existing’ patient records, following the Note Summarising Protocol and practice deadlines
- Monitor task lists and take appropriate actions.
- Process incoming post, distribute to the appropriate staff / department.
- Take professional pride in the quality of your work and contribute to a positive and supportive team.
- To attend and contribute to staff meetings and development meetings constructively.

**Secondary Responsibilities**
- Participate in staff training sessions and online e-learning
- Any other administrative duties and record keeping, as required and relevant to the position, as seen reasonable by the Administration Manager / Assistant Administration Manager

This is a varied and busy position. Whilst you will not be required to undertake all the above responsibilities on your own, the successful applicant will have the ability to multitask and prioritise workload.

**Qualifications**

**Essential**

Qualified to GCSE standard or NVQ equivalent

**Experience**

**Essential**

Experience in a busy office environment

**Desirable**

Previous GP surgery / NHS administration experience (minimum two years' experience)

Experience of SystmOne (TPP)

**Skills**

**Essential**

Excellent keyboard and computer skills

Able to work in a changing environment

Highly organised and able to follow protocols accurately and efficiently

The ability to work under pressure

Great communication skills

**Personal Qualities**

**Essential**

A keen attention to detail

Team player

An understanding of confidentiality in the workplace
