Reablement Practitioner Hcc602390

We are seeking a skilled, enthusiastic, and self-motivated Reablement Practitioner to join our Adults Health and Care Reablement Service in West of Hampshire, based in Totton. As a Reablement Practitioner, you will be responsible for carrying out holistic assessments of Reablement need and planning Reablement interventions to support across a multi-disciplinary team of Health and Social Care professionals. You will have a primary focus supporting those with sensory needs and will sit within the sensory component of our wider reablement team but will support broader functions including hospital discharge activity and prevention of admission.

Full induction, supervision, performance reviews, Continuing Professional Development opportunities and professional, clinical, managerial and peer support will be available to support you. As you will need to travel around Hampshire, being able to drive and have access to a car is a necessity.
