**Recovery Coordinator - Part-time (26.5 hours per week)**

An exciting opportunity has arisen for a Recovery Coordinator within our Northallerton team at North Yorkshire Horizons.
- North Yorkshire Horizons are the commissioned provider of Drug and Alcohol treatment services for adults in North Yorkshire. We accept referrals from hospitals, GP surgeries other professional agencies across the county, as well as self-referrals from individuals. We deliver a wide range of structured clinical and psychosocial interventions for our service users, along with aftercare support to help them maintain sustained recovery from problematic substance use.
- **About the role**

The role of a Recovery Coordinator is to manage a service user caseload, devising, implementing, and reviewing ongoing recovery plans and treatment options, liaising closely with the full range of workers and skills within the service team, including family members and other professional support where appropriate. You will help set goals for recovery and help the service user achieve their goals in a safe and supportive way.
- Key Duties:

- Manage a caseload, devising, implementing, and reviewing ongoing recovery and treatment plans.
- Facilitate and promote individuals’ progression from initial engagement, through structured treatment, to self-sustained recovery.
- Reduce substance-related harm to individuals and the wider community.
- Working collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
- Work within the community in GP surgeries, Mental health services and other partner agencies.

**About you**

Working across the Hambleton and Richmondshire districts of North Yorkshire, you will be required to work as part of a team to provide recovery planning and case management for adults impacted by problematic substance use (Drug and Alcohol) at different stages of their recovery journey, post assessment by providing structured treatment, support and appropriate psychosocial interventions.

The applicant is expected to be a driver, and have position of a suitable vehicle, with business insurance cover.
