- Salary: £27,218 -£33,133
**About Us**
The Lantern Trust is an independent registered charity that was founded in 1983 to help vulnerable and marginalised people in the Weymouth and Portland area. The Lantern Trust aims to bring a lasting change in people’s lives through our dedicated service; meeting people’s practical, emotional, and personal aftercare needs. Our ethos is one of social inclusion, and we aim to break cycles of disadvantage and dependency.
**Context**
An exciting opportunity to be a part of the development of wellbeing hubs across Dorset, as part of the universal offer of wellbeing support, an element of the mental health transformation programme for adults.
Hope House in Weymouth will host a wellbeing hub in Dorset and is being developed through partnership involving the NHS, Community Action Network, Help and Care and Prama.
The hubs will support people to take the steps that matter to them, by connecting them to what they might need, at the right time and in the right place, ultimately empowering people to make their own choices and moving towards full citizenship within their communities. Citizenship in this context means having a voice, a sense of belonging, purpose, identity and equal access to the rights and opportunities that should be available to all.
A community based service with open access for the community. The purpose is for people to be able to gain access to services and support at a time that suits them and that the hubs are able to respond to in a swift and dynamic way. The emphasis is to provide support at the earliest opportunity to move towards preventing crisis’ happening in people’s lives with the ability to flex and tailor its service offer from low level information and signposting to personalised, coproduced wellbeing plans with multi-disciplinary support for recovery and sustainable wellbeing opportunities.

**Main Purpose of the role**
To support people who experience mental health challenges to build the lives they wish to live and have access to the same opportunities and rights as other citizens. By building a support offer which covers all aspects of someone’s life, be it mental, physical, and emotional.
To create interactions that provide people with a sense of being heard and understood, as well as valued and respected as individuals, enabling people to have choice and develop a sense of agency.
To employ a trauma-informed approach to enable and support access to other services and sources of support, particularly for those who experience barriers in this process and may have had difficult experiences previously.
To collaboratively develop a plan going forward taking a holistic approach, offering choice and utilising tools such as the ‘My Wellbeing Plan’ and Dialog+, while recognising that this may not be a linear process.
To act as a bridge between services, enabling understanding and transparent communication, managing expectations, and advocating for people at multi-disciplinary/interface team meetings and collaborative triage.
To work effectively to promote psychological safety for all.
**Responsibilities / Tasks**
1. Creating a warm and welcoming first point of contact at the hub, building rapport and relational safety to create a space where people feel safe and welcome.
2. To consider the person as the expert on themselves, utilizing listening skills to enable person-centred interactions and outcomes.
3. Employ a strengths-based approach, acknowledging the strengths and skills an individual brings, supporting self-management while recognising that capacity for this may change over time.
4. Enabling and supporting choice, making decisions collaboratively with the person and being guided by their preferences. Advocating for the person at multi-disciplinary/interface team meetings and collaborative triage, knowing when to escalate concerns.
5. Promoting the range of local resources, groups and opportunities to encourage positive wellbeing experiences within the community.
6. To walk alongside people and support warm transfers to relevant service support with transparency and clarity about what someone may expect from these services. Encouraging the person to advocate for themselves and act as independently as possible.
7. To tailor communication and explanations to ensure that people are empowered to make informed choices, enabling engagement with accessible sources of education and information, including the translation of clinical language.
