Community Matron

To work as a Community Matron managing a caseload for a 'High risk' group of people in the Primary Care setting with complex needs and predominantly long term conditions. The post holder will provide a holistic approach which includes holding overall responsibility for management of their health and social care needs.

To deliver care in accordance with national and local priorities, Organisation Policies, procedures and guidelines in accordance with the Nursing and Midwifery Council.

The Community Matron will work collaboratively with GP's, practice/district nurses, social care colleagues, medicines management and Allied Health Care professionals. They will lead and facilitate a patient focused, coordinated case management approach across primary and secondary care for people who are most vulnerable to and at high risk of repeat admissions to hospital.

The post holder will participate in and influence efforts across health and social services to shape multi-disciplinary case management approaches designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in the least intensive setting possible.
