**Job Title**:Care Navigator**

**Reports to**: Care Navigation Team Manager

**Job summary**:
**Shifts to be decided - Monday to Friday between hours of 8.00 and 18.30**

Main duties include
- SIGNPOSTING patients to access appropriate services or healthcare professionals in a courteous, efficient and effective way.
- WELCOMING patients and visitors to the Health Centre.
- SUPPORTING the wider health centre team, including clinicians and admin staff to ensure first class service.
- COMMUNICATING with health care professionals, patients and non-clinical colleagues to ensure the smooth running of the health centre.

**Confidentiality**:

- While seeking treatment from the health centre, patients entrust the staff with, and allow them to gather sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- You will be required to undertake regular GDPR and data protection training and maintain confidentiality at all times.

**Experience**: Previous experience within a front facing service is essential. Previous experience within a health care based service is desirable. Experience of working within a fast paced, changeable environment would be beneficial.

**Skills**: Excellent customer service skills are essential. Excellent telephony skills are essential. Basic computer skills are desirable, however full training will be given on the health centres IT systems.

**Job Types**: Part-time, Permanent
Part-time hours: 18-30 per week

**Salary**: £10.87 per hour
