Christchurch Day Hospital are looking for a dynamic 1 WTE ACP (Nursing/AHP background) to join the team in an autonomous role working with older people in both the day hospital clinic environment and within the patients homes. This is an exciting opportunity whereby you will be expected to autonomously assess, set treatment plans, prescribe and manage acute episodes of illness, deconditioning and longer term chronic conditions. You will work as part of a MDT assessing patients with falls and other neurological/Older persons conditions and will be a resource for knowledge within the wider MDT. You will be expected to autonomously follow up blood results/radiological investigations and act upon them accordingly and will be expected to autonomously see patients in an out-patient clinic environment.

You will have demonstrable experience in working autonomously at this level and will be knowledge in frailty and the process of comprehensive geriatric assessment process. You will also need to have transport to be able to assess patients in their own homes where required.

There will be the requirement to cover weekends/Bank Holidays within the development and planning of this role and also there will be the expectation for you to work flexibly and provide cover for Ward 9 community bed ward on the Bournemouth Hospital Site.

This role requires a high level of autonomy and you will be required to utilise your advanced assessment and decision making skills, critical thinking and clinical reasoning with confidence and competence to independently manage a caseload of patients in a variable and evolving service.
