The Wokingham Community Older Adults Mental Health Service is a multi-disciplinary team with specialist skills in caring for older people with mental health needs in the community. We are looking for an enthusiastic, caring and highly motivated Assistant Practitioner to join our friendly and innovative Home Treatment Team (HTT).

Your base would be Wokingham Community Hospital where we have access to free parking facilities, staff room and a friendly and welcoming team.

The post holder will have two years’ experience in mental health services with the relevant clinical experience and either have or be willing to undertake NVQ 4 / further training relevant to the role.

The post holder will be well supported and guided by the Clinical / HTT Lead and Mental Health Practitioners within the Home Treatment Team and expected to operate within the overall framework of the team’s policies and procedures. Support is provided through 1:1 and group supervision. The successful applicant must display a passion & understanding for working with Older People.

The normal rota hours are either 9-5pm or 11-7pm, including a weekend shift 9-3pm once a month (Appropriate supervision will be provided for weekend cover). There will be the opportunity for NHSP overtime to cover sickness and leave.

T he post holder will under supervision hold a caseload of patients with low level needs with the aim of improving wellbeing and providing a therapeutic relationship.

In addition, the post holder will support the delivery of excellent care provided to our HTT caseload, supporting the implementation of intensive care and treatment in the homes of people with functional mental health problems, as well as people with dementia.
