We have a great job opportunity to join a focused and dynamic team who are involved in the assessment and development of a wider range of services to meet the current and future needs of adults.

We are looking for a Social Worker to join Community Team.

**Location**:Greater Manchester

**Pay rate -£30PH**

**Your key responsibilities will include**:

- Assessment and developing and arranging appropriate packages of care/services for service users, their families and carers, and providing professional support where required. Reviewing any packages of care provided.
- To adhere to policy, procedures and best practice and ensure that Adults are kept safe and adhere to adults safeguarding policy, practice and legislation as required.
- To attend any multi disciplinary meetings to represent clients to deliver the best outcomes possible.
- Manage and be accountable for their own work, ensuring all records and plans are up to date and clear on the appropriate systems.
- A knowledge of Liquid Logic would be beneficial but if not, training will be provided.

**Essential Requirements**:

- SWE Registered
- Must have at least two years of post-qualifying experience working with adults in a statutory setting
- Require Driving License
